Complete the Form

After clicking the "Book Now" link below, fill out the patient information form and choose your appointment time.  

Make a Payment

Once the form is complete, you'll be able to make payment using your credit card.

Check Your Email

Once payment is received, you'll receive a link along with a password to enter the virtual waiting room.

Login to the Portal

Use the link from the email to enter the virtual waiting room. Enter the password provided in the email.

Before booking your appointment, please be sure to read the telehealth appointment policies.

If you are having a medical emergency, dial 911 or go directly to your nearest emergency room.

Telehealth Appointment Policies

It is our goal to ensure you have the best possible telehealth experience. In order for that to occur, we want to ensure that you have all of the information you need to get started and prevent any issues as much as possible. Please read the information below very carefully.

Location Requirements

It is against Georgia law for physicians to provide telehealth services to patients who are located outside of the state. By using our telehealth services, you attest to being physically located within the state boundaries. For verification, we ask that you put the address of your current physical location when you complete the patient intake form.

We treat more than just what is listed here. The conditions listed are just a few of the most common conditions we treat via telehealth. If you have a question concerning telehealth services or conditions we treat, please submit them via the contact form on the appointments page.

Services Not Available Through Telehealth

Not all medical conditions can be treated appropriately using telehealth or telemedicine. In addition, there are some medical services we are not able to provide via telehealth, including:

  • Prescriptions or refills for scheduled medications, such as

    • ADHD stimulant medications (Adderall, Ritalin, Vyvanse, etc.)​

    • Benzodiazapines (Valium, Xanax, Ativan, Klonipin, etc.)

    • Testosterone

    • Opioid pain medications (Hydrocodone, Oxycodone, Morphine, etc.)

    • Suboxone

    • Ketamine

    • Soma

  • Visits that require a form to be filled out by a physician

  • Treatments for emergency conditions 

  • Treatments that require an invasive or surgical procedure

The list of services we do not provide may include services not included here. 

Payment & Insurance Policy

Payment for telehealth services is required in full prior to being seen. We are an out-of-network provider. We do not submit claims to insurance companies, including claims for Medicare advantage plans. If requested, we can usually provide you a document that contains the information you need to submit a claim to your insurance company. Your insurance company may reimburse you for all or part of the cost of your telehealth visit. We recommend checking with your insurance company to determine if you  may be reimbursed. 

Payment for telehealth visits only include the cost of the visit. In the event that certain test are required, you will be responsible for those costs. We contract with local laboratories everywhere in the state of Georgia to ensure you get the lowest possible prices on laboratory testing. You are also responsible for the cost of any prescription medications. All prescriptions are submitted to the pharmacy of your choice. 

All sales are final. We do not offer refunds. The required payment includes the allocation of our physicians' and other providers' time for your visit. In the event that you are late or miss your appointment, your payment will not be refunded. 

Device Requirements

For your appointment, you will need to have access to a private place and an electronic device equipped with a camera and high speed internet. Black Tie Health™ is partnered with Doctor.com to provide you with access to a videoconferencing software free of charge. Our software is secure and undergoes routine audits and security checks to ensure that it complies with all HIPAA regulations.

Please ensure that your device and web browser is compatible with our telehealth software prior to booking an appointment. Your payment will not be refunded because of device issues, compatibility issues, or other preventable reasons.

The web browsers that are known to function properly with our telehealth software is below. You will need to allow your web browser to access your camera and microphone.

Please note, the only browser compatible with iPhones, iPads, and iPod touch is Apple Safari. All other devices should use Google Chrome to ensure maximum compatibility.
